For this case, you will read the vignette carefully and complete a service plan for the client, using the prompts below.
Case #1 Vignette:
Kim is a 17-year-old Latina who identifies as lesbian. She was recently kicked out of her house after her mother found a love letter she had written to a female schoolmate. Kim’s mother is a devout Catholic who believes that homosexuality is a sin. Kim has no relationship with her father. He left Kim’s mother when Kim was an infant and his whereabout are unknown. Kim has been anxious, can’t sleep, and stays with friends or sleeps in an alley near her high school. She has been taking showers at a friend’s house and walking to her school, James A. Garfield High School in East Los Angeles. She loves school and wants to get a graduate degree in veterinary medicine, but she has no place to call home, no job, and is running out of spending money.
You will turn in a 4-5 page paper to answer the following:
Provide two case management goals and objectives of the selected client in your service plan.
Your service plan should include what you will do during the three phases of the CM process.
Apply the 4 components of the Bergs Strength Assessment to your client
Develop a list of strategies and interventions based in research to treat the case management needs of this client. Include a summary of at least two different interventions. Incorporate at least one scholarly journal article for each intervention to provide support.
Distinguish between direct interventions and care coordination/advocacy that will be used in the treatment of this case management client.
Describe and address ethical considerations applied in the case management treatment of this client.
Describe the cultural considerations will you be applying when working with your client
